A penoplasty by fat injection requires a preoperative check-up which will be prescribed during the consultation with Dr. Diacakis plastic surgeon in Paris at least 15 days before the operation.
It is recommended before a penoplasty operation to stop smoking two weeks before the operation and not to take aspirin. 

The operation is usually performed under general anaesthesia. Lengthening and thickening can be done at the same time. 

• Enlargement penoplasty :  

Removal of fat (from the inner side of the thighs, abdomen or pubis) then filtered or centrifuged to be finally deposited under the skin of the penis (in a well-defined space) thanks to a very fine instrumentation particularly adapted to this technique.
The glans cannot be thickened with fat. It can be thickened with hyaluronic acid. 

• Lengthening penoplasty : 

The treatment of the suspensory ligament for penis lengthening makes it possible to gain at least one centimetre in erection and 3 to 4 cm at rest thanks also to the thickening of the penis and the skin flap of advancement. The latter will leave a vertical scar, hidden in the pubic hair.  

There is little pain after a penoplasty.
Several appointments for the follow-up of the operation will be established with the plastic surgeon in Paris. 

• Enlargement penoplasty : 

The penis is very swollen for at least 2 weeks. It is then often difficult to decalate it. Showering is allowed from the following day. No bandage is necessary because of the risk of compression.
A preventive antibiotic treatment is prescribed. As a rule, professional activities can be resumed after 48 hours.
Sexual rest is necessary for 4 to 6 weeks. 

• Lengthening penoplasty : 

Local, simple care of the scar (betadine disinfection and dressing) is to be done every two days until healing (for about two weeks).
The stitches are resorbable.
A preventive antibiotic treatment is prescribed. As a rule, work can be resumed after ten days.
Sexual rest is recommended for 4 to 6 weeks.

Enlargement penoplasty : 

The final result is obtained in 3 to 6 months on average.
In the majority of cases, it increases the circumference of the penis by approximately 2 to 4 centimetres both at rest and in erection. Weight variations can affect the result.
Possible complications are those of lipofilling (fat tissue grafting) with the risk of resorption of the fat transplant, fatty indurations or deformations. They will disappear in a few weeks, spontaneously or with the help of localised corticoid injections or by lipoaspiration. 

Lengthening penoplasty : 

The result is visible after one month and the scar is hidden in the pubic hair.
The partial section of the suspensory ligament associated with the skin flap allows an increase in the length of the penis of about 2 to 5 cm at rest and in erection we can sometimes expect 1 cm.
There is no direct effect on erection or libido.
